When submitting a specimen for testing, you must print (or order) the appropriate form, fill it out completely, submit the form and specimen following the appropriate rules and regulations for packaging to:

You may print a copy of the Lab Submission Form (pdf) for your use.

Laboratory Submission Continuation Form
You may print a copy of the Lab Submission Continuation Form (pdf) for your use.

Owner-Collected Samples Lab Submission Form
You may print a copy of the Owner Submission Form (pdf) for your use. For Q Fever testing submit 2 oz milk (keep refrigerated or frozen) or serum (keep refrigerated).

Equine Viral Arteritis (EVA) Form
You may print a copy of the EVA Test and Vaccination Certificate Form (pdf) for your use.
